Moving locally
If you move to another town in Andalucia, then the process of advising the Health Authorities is very straight-forward.
Moving to another Autonomous Region
If you move from Andalucia but are still in Spain, the process is a little more involved because each Autonomous region has it's own health card system so you will need a new medical card.
Leaving the Country
Should the time come to leave Spain, it will be necessary to deregister by handing in your medical card and ensuring any pending hospital appointments have been cancelled.

Healthcare in Spain
